COPD-X Checklist

The essential elements of The COPDX Plan: Australian and New Zealand Guidelines for the management of COPD are contained on a two-page A4 flipcard.

The simple tool allows general practitioners and other health professionals to follow best practice when diagnosing and managing patients with COPD.

COPD Action Plan

The concept of self-management plans for patients with COPD is derived from their success in asthma management , indicating doses and medications to take for maintenance therapy and for exacerbations. Instructions for crises are often also included.

The Australian Lung Foundation, through its COPD National Programme, has developed a COPD Action Plan for use by patients and written by the patient’s health care professional.

Patient Questionnaire

Five simple questions:
  • Do you cough several times most days?

  • Do you bring up phlegm or mucous most days?

  • Do you get out of breath more easily than others your age?

  • Are you over 40 years old?

  • Are you a smoker or ex-smoker?

People are encouraged to speak to their doctor if they answer yes to any three of these questions.

Pulmonary Rehabilitation Toolkit

The Australian Lung Foundation, together with the Australian Physiotherapy Association, has launched the Pulmonary Rehabilitation Toolkit.

This web-based Toolkit is available at www.pulmonaryrehab.com.au.

The Toolkit has been developed to help health professionals establish pulmonary rehabilitation programs in metropolitan, regional and rural settings, throughout Australia.

A summary card and checklist that highlights the key features and minimum requirements of a pulmonary rehabilitation program is available at
